Community Health Program Supervisor

THE POSITION
The Hennepin County's Human Services and Public Health Department is seeking two Community Health Program Supervisors. The first position, the Outreach Grant Supervisor will support grants that are focused on outreach and community-based activities. The second position, the Clinic Grant Supervisor, will support staff whose activities focus on clinic-based grant work that includes both the Disease Intervention and the PrEP (Pre-exposure prophylaxis) Programs. Both positions will manage, develop, implement, and evaluate HIV/STD Prevention Services strategies and supervise a team of employees.

Location and hours:
These positions are located at the Red Door Clinic at 525 Portland Avenue, Minneapolis, MN 55415. Currently due to COVID, the clinic is open Monday, Tuesday, Wednesday, and Friday 8 a.m. - 4 p.m., and Thursday 10 a.m. - 4 p.m. Hours and schedule are flexible based on planned outreach activities and the scope of responsibilities of these positions. Occasional weekend hours may be required depending on special events.

In these positions, you will:  
Outreach Grant Supervisor:
  • Supervise a dynamic team of HIV outreach and prevention services staff who provide HIV/STI public health program services including, HIV testing in clinical and community settings, HIV care linkage and navigation services, and population specific programming to communities who are disproportionately affected by HIV.

Clinic Grant Supervisor:
  • Supervise a dynamic team of HIV Prevention Services staff who are counseling and interviewing individuals (with emphasis on partner notification) who are diagnosed with HIV, syphilis, chlamydia and gonorrhea (under delegation authority from the Minnesota Department of Health). This team also includes staff who are delivering our PrEP services which includes education, promotion, program intake, benefits navigation and adherence support.

Both Supervisors:
  • Manage operations of Outreach and Prevention Services programs and the implementation of all contracted grant duties and activities including: analysis of grant and contractual obligations for Centers for Disease Control guidelines, Minnesota Department of Health guidelines, and Ryan White federal guidelines.
  • Coordinate planning workgroups for grant program development.
  • Write and oversee grant application processes for all HIV/STI prevention programs; including budget development and coordination of staff who contribute written sections of applications.
  • Prepare and manage annual budgets for multiple programs. Monitor expenditures, revenue, program invoicing, and prepare required financial reports for various funders including state and federal entities. 
  • Work with a diverse team to develop team goals, assess workforce needs, establish performance outcomes, maximize performance results, plan for implementation of programs, and prepare schedules for completion of work.
  • Facilitate continuous process improvement of systems and programs by determining needs and improvements; develop plans, policies, and procedures; and evaluate outcomes for programs and services.
  • Participate as an active member of the Public Health Clinic management team and the larger Public Health managers/supervisors team; serve as representative of the department, service area, and Public Health Clinic to internal and external audiences; may serve in key role for Public Health Emergency Preparedness Incident Command.
 
Need to have:
  • One of the following:
  • Bachelor's degree or higher in public health, public administration, community health, environmental health, nursing, behavioral/social science or other field appropriate to the position and two years or more of professional work experience in a related program area.
  • Six years or more of professional work experience in a related program area.
  • Access to reliable transportation to travel within the Twin Cites metro area.

Nice to have:   
  • Considerable knowledge of public health and HIV/STD prevention programs.
  • Experience:
  • Providing leadership to a team or peers; supervisory experience; ability to provide work direction to professional and paraprofessional staff.
  • Conducting sexually transmitted disease investigation interviews and partner notification services.
  • Delivering PrEP services, education, and promotion.
  • Developing plans, goals, resources, and managing public health strategies and programs.
  • With development, writing, and management of public health grants.
  • With social marketing, HIV/STD prevention advertising, and the use of social media.
  • Working on multidisciplinary workgroups and management teams.
  • Ability to: 
  • Manage budgets, set goals and objectives with timelines to achieve results, exercise independent judgment and initiative to accomplish work.
  • Prepare and submit accurate and concise reports; effectively communicate in verbal and written form.
  • Foster an environment that recognizes and respects cultural, lifestyle, and socioeconomic differences.
